Booking and client accounts through IntakeQ
The appointment and client login pages embed IntakeQ, the practice’s booking and client portal provider. Information you enter into those forms is submitted to IntakeQ. The website does not separately save copies of your appointment details, forms, or login credentials.
When you visit a page with an IntakeQ widget, your browser connects to IntakeQ. Its service may process technical information and use cookies or similar technologies to operate booking and login. Review the privacy information presented by IntakeQ before submitting information.
Other website information
The website does not include advertising pixels or analytics scripts added by the practice. Hosting, access-control, and security services may use cookies or process technical information such as your IP address and browser details.
Email inquiries
Email links open your own email service. This website does not send a message on your behalf. Your email provider processes messages under its own terms, and a message you send is received in the practice’s email account.
Use email for scheduling and general questions only. Ordinary email may not be secure. Do not send medical records, symptoms, diagnoses, medications, or other sensitive information. Use your IntakeQ client account for clinical information.
Clinical privacy information
This website privacy page is not a Notice of Privacy Practices for your health records. The practice’s clinical Notice of Privacy Practices is not yet available on this website. It will need to be finalized and provided before care begins. Please use email only for scheduling or general questions in the meantime.
